Family or household size (required)

Use the definition in your health center's board-approved policy. Whole numbers only. Households larger than eight use the official additional-person increment.

Annual household income (required)

Use the income definition and documentation rules in your health center's approved policy. Dollars and cents are accepted.

How the federal ranges work

The guideline sets the range. Your approved schedule sets the charge.

At or below 100% FPG

A full discount applies, or an approved nominal charge, as defined in the health center's board-approved policy.

Above 100% through 200% FPG

Partial discounts apply through graduated pay classes. Each health center determines its own class structure and charges.

Above 200% FPG

No sliding-fee discount is required under the Health Center Program. Other organizational assistance may still be available.

Actual fees and pay classes come from the health center's approved sliding fee discount schedule, not from the poverty guideline itself.
